A Study of Correlation of Spontaneous Bacterial Peritonitis(SBP) and Child Turcotte PUGH(CTP) Score

Abstract :

ACKGROUND : Spontaneous Bacterial Peritonitis is defined as absolute level of PMN in ascitic fluid more than 250/ml. It is the most frequent cause of infection and associated with high mortality. CTP score is used to assess the prognosis of cirrhotic patients. AIMS & OBJECTIVES : To study the incidence and prognosis of SBP in cirrhotic patients with correlation to CTP score METHOD : Seventy Five(75) patients of chronic liver disease above 12 years of age irrespective of sex and etiology admitted between august 2012 to august 2013 were studied retrospectively and their ascitic fluid analysis and CTP score using bilirubin,prothrombin time, serum albumin, grade of ascites and encephalopathy were assessed and correlated. RESULTS : In our study, 25.3% of the patients were diagnosed with SBP. Of these patients with SBP, 78% of them had CTP score 10 or more. Thus 39% patients with CTP >=10 had SBP as compared to 10% incidence of SBP in other patients. Five of SBP patients expired. 4 of them had CTP >=10 thus amounting to higher mortality in this subgroup. CONCLUSION : Incidence of SBP was higher in patients with CTP score >=10(stage C.) and was associated with poor prognosis and higher mortality rate. Such patients need more intensive care and management
