FEBRILE SEIZRUES: DEMOGRAPHIC, CLINICAL AND ETIOLOGICAL PROFILE OF CHILDREN ADMITTED WITH FEBRILE SEIZURES IN A TERTIARY CARE HOSPITAL

Abstract :
<p> <b><span lang="EN-IN" style="font-size:14.0pt;line-height:200%;font-family:"Times New Roman","serif"">Background: </span></b><span lang="EN-IN" style="font-size: 12pt; line-height: 200%; font-family: "Times New Roman", serif;">Feile seizures are a common occurrence in the paediatric population and a cause of great parental anxiety and concern. This study was conducted to evaluate the clinical profile of children presenting with feile seizure in a teaching hospital.</span></p> <p class="MsoNormal"><b><span lang="EN-IN" style="font-size:14.0pt;line-height:200%;font-family:"Times New Roman","serif"">Methods</span></b><span lang="EN-IN" style="font-size:14.0pt;line-height:200%;font-family:"Times New Roman","serif"">: </span><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family: "Times New Roman","serif"">The hospital-based prospective study was conducted in the department of Paediatrics, </span><span lang="EN-IN" style="font-size:12.0pt;line-height: 200%;font-family:"Times New Roman","serif";mso-fareast-font-family:"MS Mincho"">Hindu Hriday Samrat Balasaheb Thackrey Medical College (HBTMC) & </span><span lang="EN-IN" style="font-size: 12pt; line-height: 200%; font-family: "Times New Roman", serif;">Dr R.N. Cooper Hospital, Mumbai</span><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family:"Times New Roman","serif""> from July 2013 to June 2014, to determine the demographic, clinical and aetiological profile of paediatric patients admitted with feile seizures, and to compare with similar studies</span><span lang="EN-IN" style="font-size:14.0pt;line-height:200%;font-family:"Times New Roman","serif"">. </span><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family: "Times New Roman","serif"">Ethical clearance was obtained from the Institutional Ethics Committee prior to starting the study. </span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size:14.0pt;line-height:200%; font-family:"Times New Roman","serif"">Diagnostic criteria as per AAP Clinical Practice Guidelines,2008 were used.</span><span lang="EN-IN" style="font-size: 12.0pt;line-height:200%;font-family:"Times New Roman","serif""><o:p></o:p></span></p> <p class="MsoNormal"><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size: 14.0pt;line-height:200%;font-family:"Times New Roman","serif"">As defined by AAP feile seizure occur in absence of intracranial infections, metabolic disturbances or history of afeile seizures,and are classified as simple or complex. Patients clinically diagnosed as a case of feile seizure were included in study, while patients with prior episodes of afeile seizures, abnormal neurodevelopment and not meeting the age criteria were excluded. </span><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family:"Times New Roman","serif"">Details of demography, clinical history including duration of fever, type of seizures, associated morbidities, and family history of feile seizures/epilepsy were collected </span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size: 14.0pt;line-height:200%;font-family:"Times New Roman","serif"">using a predesigned proforma</span><span lang="EN-IN" style="font-size:12.0pt;line-height: 200%;font-family:"Times New Roman","serif"">. General and systemic examination findings were noted. </span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size:14.0pt;line-height:200%; font-family:"Times New Roman","serif"">Collected data was analyzed using Microsoft excel.</span><span lang="EN-IN" style="font-size:12.0pt;line-height: 200%;font-family:"Times New Roman","serif""><o:p></o:p></span></p> <p class="MsoNormal"><b><span lang="EN-IN" style="font-size: 12pt; line-height: 200%; font-family: "Times New Roman", serif;">Results: </span></b><span lang="EN-IN" style="font-size: 12pt; line-height: 200%; font-family: "Times New Roman", serif;">Total 50 cases meeting inclusion criteria got enrolled in the study period. </span><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family:"Times New Roman","serif"">Majority of cases (72%) were less than 24 months of age at presentation.<o:p></o:p></span></p> <p class="MsoNormal"><span lang="EN-IN" style="font-size:12.0pt;line-height:200%; font-family:"Times New Roman","serif"">28 cases (56%) were male and 22 (44%) were female. 84% presented with simple feile seizures and 16% had complex feile seizures. Family history of feile seizures was found in six of the cases (12%) Physical examination revealed pallor in 30 (60%) cases of feile seizures. Acute Viral Illness without focus and URTI were most commonly associated with feile seizures (92%). Mean temperature is 38.35<u>+</u> 1.9<sup>o</sup> Celsius and median temperature is 38.5<sup>o</sup> Celsius.<o:p></o:p></span></p> <p class="MsoNormal" align="left"><b><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family:"Times New Roman","serif"">Conclusion:</span></b><span lang="EN-IN" style="font-size:12.0pt;line-height:200%;font-family:"Times New Roman","serif""> </span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size:10.0pt; line-height:200%;font-family:"Times-Roman","serif";mso-bidi-font-family:Times-Roman">In conclusion, m</span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size: 14.0pt;line-height:200%;font-family:"Times New Roman","serif"">ajority of patients were male,</span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size: 10.0pt;line-height:200%;font-family:"Times New Roman","serif";mso-fareast-font-family: Calii;mso-fareast-theme-font:minor-latin;mso-ansi-language:EN-US"> </span><span style="font-size:12.0pt;mso-bidi-font-size:10.0pt;line-height:200%;font-family: "Times New Roman","serif";mso-fareast-font-family:Calii;mso-fareast-theme-font: minor-latin;mso-ansi-language:EN-US">predominantly of age below two years</span><span style="font-size:12.0pt;mso-bidi-font-size:14.0pt; line-height:200%;font-family:"Times New Roman","serif""> <span lang="EN-IN">and admitted with first episode.</span></span><span lang="EN-IN" style="font-size: 12.0pt;mso-bidi-font-size:10.0pt;line-height:200%;font-family:"Times New Roman","serif"; mso-fareast-font-family:Calii;mso-fareast-theme-font:minor-latin;mso-ansi-language: EN-US"> </span><span style="font-size:12.0pt;mso-bidi-font-size:10.0pt; line-height:200%;font-family:"Times New Roman","serif";mso-fareast-font-family: Calii;mso-fareast-theme-font:minor-latin;mso-ansi-language:EN-US">Simple feile seizure was the commonest variety. </span><span lang="EN-IN" style="font-size:12.0pt;mso-bidi-font-size:14.0pt;line-height:200%;font-family: "Times New Roman","serif"">Parents should be properly councelled regarding prophylaxis and benign nature of illness. As most of the cases are of viral etiology , antibiotics should be used with discretion</span><span lang="EN-IN" style="font-size:14.0pt;line-height:200%;font-family:"Times New Roman","serif"">.<o:p></o:p></span></p>
